How this website handles the information you give it.
What we collect
When you send an appointment request, the website collects what you type into the form: your name, phone number, email address, your preferred day and time, and any note you add. It is emailed to our office and kept on the website so we can schedule you. Please do not put medical details in the form; we go over your health with you in person.
Patient forms
Our online health record and auto accident forms are provided by ReviewWave, a patient-forms service, and what you enter there goes to that service under its own privacy and security terms.
What we do with it
We use your information only to contact you and schedule your care. We do not sell, trade or give it to anyone else, except trusted providers who help us run this website and our office and who agree to keep it confidential.
Cookies
This website does not use advertising or tracking cookies. The appointment form may use a small technical cookie so it works.
